    When the collision happened, and where — the road, the junction, the premises.

    The report number

    The police or incident report number, if one was made at the scene.

    The other parties

    Their names, and their insurance carriers if you have been given them.

    Anything you took of the scene, the vehicles, or your injuries.

    Your treatment

    Where you have been seen so far, and what you were told there.
